There is a question that many patients on prednisolone eventually ask their doctor, often with a mixture of relief and unease: If I am feeling so much better, why do I keep getting sick? A skin infection that lingers. A cold that settles into the chest. A minor cut that takes longer than expected to heal. These experiences can feel contradictory — even alarming — when the very medication responsible for them is also reducing joint swelling, calming a flare, or protecting vital organs from autoimmune attack.

The answer lies in a biological paradox built into the drug itself. Prednisolone does not discriminate between the immune activity that damages your own tissues and the immune activity that guards you against outside threats. To understand why, it helps to look closely at what prednisolone actually does inside the body.

The Immune System Is Not a Single Switch

Many patients understandably think of immunity as a unified force — either strong or weak. In reality, the immune system operates through dozens of overlapping cellular networks, each with a specific role. Neutrophils patrol the bloodstream and rush to sites of bacterial invasion. T-lymphocytes coordinate targeted attacks against viruses and abnormal cells. Macrophages engulf pathogens and signal the rest of the system when danger is detected. Natural killer cells identify and destroy infected or cancerous cells before they spread.

In autoimmune and severe inflammatory conditions — rheumatoid arthritis, lupus, inflammatory bowel disease, asthma, and many others — portions of this system become dysregulated. They begin attacking healthy tissue as though it were foreign. Prednisolone is prescribed precisely because it is a potent suppressor of these runaway immune signals.

The difficulty is that the same cellular machinery driving autoimmune damage is also the machinery your body depends upon to fight real infections.

What Prednisolone Does at the Cellular Level

Prednisolone belongs to the glucocorticoid class of corticosteroids. Once absorbed, it enters cells and binds to glucocorticoid receptors in the cytoplasm. The resulting complex travels into the cell nucleus, where it alters the expression of hundreds of genes — including those governing the production of inflammatory cytokines such as interleukin-1, interleukin-6, and tumor necrosis factor-alpha.

This mechanism is why prednisolone is so effective. By suppressing cytokine production, it quiets the inflammatory cascade before it can cause further tissue destruction. Swelling recedes. Pain diminishes. Organ function may stabilize or improve.

However, many of those same cytokines serve a critical defensive purpose. Interleukin-1 and interleukin-6, for example, are central to the fever response — the elevated body temperature that slows bacterial replication and signals the immune system to mobilize. When prednisolone dampens these signals, the body's early-warning system becomes muffled.

Beyond cytokines, prednisolone reduces the number and function of circulating lymphocytes, impairs neutrophil migration to infected tissue, and decreases the activity of macrophages. In practical terms, this means that bacteria, fungi, and certain viruses encounter less resistance than they would in an immunologically intact individual.

Why Higher Doses Carry Greater Risk

The degree of immunosuppression is not constant. It scales, broadly speaking, with dose and duration. Patients taking low-dose prednisolone — often defined as 10 milligrams or less per day — face a measurably different risk profile than those on moderate or high doses for extended periods.

At higher doses, the suppression of neutrophil function becomes clinically significant. Neutrophils are among the body's first responders to bacterial infection, and when their migration is impaired, infections that a healthy immune system would contain rapidly can instead spread. This is one reason that physicians monitor patients on higher doses with particular vigilance and may recommend prophylactic medications — such as trimethoprim-sulfamethoxazole to prevent Pneumocystis jirovecii pneumonia — in specific clinical scenarios.

Long-term use introduces an additional complication: the cumulative effect on immune memory. Repeated or sustained corticosteroid exposure can blunt the adaptive immune response, meaning the body may not generate as robust a defense even after the drug is eventually tapered.

The Psychological Weight of Feeling Better but Being More Vulnerable

For many Americans managing chronic inflammatory conditions, the emotional dimension of this paradox is as challenging as the biological one. There is genuine relief when prednisolone begins working — when the joint pain eases, the rash fades, or breathing becomes less labored. That relief can make it difficult to remain vigilant about infection risk, particularly when the body no longer signals distress in the ways it once did.

The dampening of fever and localized inflammation means that infections may present atypically in patients on prednisolone. A serious bacterial infection might produce only a low-grade temperature. An infected wound may show less redness and swelling than expected. This masking effect is clinically important: it means patients cannot rely solely on how they feel to gauge whether an infection is developing.

Healthcare providers often describe this as the need for a heightened index of suspicion — a deliberate alertness to subtle changes that might otherwise be dismissed.

Practical Steps to Reduce Infection Risk

While the immunosuppressive effects of prednisolone cannot be entirely eliminated without reducing the dose — a decision that belongs to the prescribing clinician — there are meaningful steps patients can take to lower their vulnerability.

Stay current with vaccinations. Patients on corticosteroids should discuss their immunization status with their physician. Live vaccines such as the MMR (measles, mumps, rubella) and the varicella (chickenpox) vaccine are generally contraindicated during significant immunosuppression, but inactivated vaccines — including the annual influenza vaccine, the pneumococcal vaccine, and the COVID-19 vaccines — are typically recommended and safe. The shingles vaccine Shingrix, which uses a recombinant formulation rather than a live virus, is also appropriate for eligible patients.

Practice consistent hand hygiene. This remains one of the most evidence-supported methods of reducing infectious exposure. Alcohol-based hand sanitizers and thorough handwashing before meals and after public contact are particularly important for immunocompromised individuals.

Avoid close contact with individuals who are acutely ill. This is especially relevant during respiratory virus season. Patients on prednisolone should communicate their immunosuppressed status to family members and household contacts so that collective precautions can be taken.

Monitor wounds and skin changes carefully. Because the inflammatory response is muted, even minor skin breaks warrant attentive care. Cleaning cuts promptly, watching for signs of delayed healing, and contacting a healthcare provider if a wound appears infected — even mildly — is prudent.

Report new symptoms promptly. Fever, even a low one, deserves medical attention in a patient on immunosuppressive therapy. Unusual fatigue, respiratory symptoms, urinary discomfort, or any sign of infection should be communicated to a physician without delay. Early intervention is substantially more effective than treatment initiated after an infection has established itself.

Do not adjust your prednisolone dose without medical guidance. Some patients, alarmed by an illness, are tempted to reduce their dose independently. This can be dangerous — particularly in patients whose adrenal function has been suppressed — and should only be done under clinical supervision.

A Relationship That Requires Active Management

Prednisolone offers many patients a quality of life that would otherwise be unattainable. For individuals with severe autoimmune disease, it may be the difference between functional independence and serious disability. But it is a medication that asks something in return: ongoing awareness, careful self-monitoring, and a close working relationship with a knowledgeable healthcare team.

Understanding that your immune system is operating under altered conditions — not failed conditions, but altered ones — can shift the experience of vulnerability from something frightening into something manageable. The goal is not to fear the medication but to use it wisely, with eyes open to both its therapeutic power and its biological trade-offs.
