Patients and providers who rely exclusively on cell counts to assess infection risk may be working from an incomplete picture.
Functional assessments—including vaccination response testing for certain patients, or evaluation of immunoglobulin levels in those on prolonged high-dose therapy—can provide more actionable information than raw cell counts in some clinical contexts. Asking your physician whether functional immune assessment is appropriate for your specific situation is a reasonable and informed question.
Building a Personalized Prevention Strategy
Rather than accepting a one-size-fits-all warning, patients on prednisolone can advocate for a structured, individualized approach to infection risk management. The following framework reflects current evidence-based guidance:
Vaccination audit. Every patient on corticosteroid therapy should have a documented vaccination review. This includes pneumococcal vaccines (PCV15 or PCV20 followed by PPSV23 in appropriate sequences), annual inactivated influenza vaccine, recombinant zoster vaccine (Shingrix) if applicable, and COVID-19 vaccination with attention to booster timing. Live vaccines require careful evaluation, as they are generally contraindicated during significant immunosuppression.
Dose and duration tracking. Maintain a clear record of your cumulative corticosteroid exposure, including previous courses.
